Project Solution:

- Build a secure, HIPAA-compliant web-based application with a device-agnostic design to support the exchange of electronic medical records and physician-signed documents.

George Pollack is a healthcare technology innovator, as well as the Founder and CEO of Digiverse, Inc., a software company that focuses on solving the healthcare system’s inefficiencies when tasked with executing documents and administrative tasks to streamline patient care delivery. The company created the HIPAA-compliant DrOrdrz web application, which is dedicated to modernizing and simplifying the workflow of physicians and other healthcare providers, empowering them to spend more time doing what they are best at: providing patient treatment and care.

As a professional in the healthcare industry for over 35 years, George recognized where encumbering dysfunction existed within the vast healthcare ecosystem and how it was hindering patient care delivery and health outcomes. This breakdown is directly linked to a lack of streamlined communication between EMR systems across the entire healthcare network.

There was no unified system to support the exchange of electronic medical records (EMR) between healthcare providers, such as home healthcare agencies, medical equipment suppliers, pharmacies, physicians, and patients. There was no secure, centralized application that could collect, track, and store physician-signed documents, which are integral to initiating orders for critical medical testing and treatments. George cites examples of these challenges, stating:

“A medical group or healthcare provider may authorize visits for home care, but most of the time, that provider will not learn whether the patient was treated or if those therapy visits took place for approximately ninety days. And that is a long time in which a lot can happen to a patient. Opportunities are missed during this time frame to continuously manage patient care and reduce their cost of care.”

“Or, there are instances where a physical therapy practice will send an order for the physician to sign, but the order is wrong, or perhaps that physician who needs to sign off on the order is on vacation. In today’s digital age, it is still necessary under these circumstances to pick up the phone and call or send a fax to notify the ancillary provider of the issue. So inevitably, these orders begin to stack up, and potentially life-saving treatments are delayed. In other instances, a medical practice may have multiple claims that cannot be billed because they are unable to get physician signatures.”

Necessity Sparks Innovation

George had an idea that would become the technological solution to these administrative challenges faced by the healthcare industry. The concept was a web-based application that would serve as the operating environment, capable of running across all operating systems (OS), including Microsoft Windows or Mac, as well as on any smartphone, laptop, or tablet.

The application, called DrOrdrz, functions as middleware between electronic medical records, acting as the connective tissue between applications, data, and users, effectively enabling communication and data management.

As George explains,

“Up until now, most EMRs have been unable to communicate with each other. Implementing that functionality is very expensive; it requires an enormous amount of customization and doesn’t work as well as it should. I saw an opportunity to create a drag-and-drop environment using the Cloud and web-based applications to facilitate securing doctor signatures for any document or EMR, including orders, prescriptions, health history records, and images. This application will effectively manage those documents from a physician’s standpoint, a billing and compliance standpoint, and other user standpoints – it is designed for the entire healthcare ecosystem, encompassing not only home care but home medical equipment, radiology imaging, skilled nursing, lab, assisted living, hospital, health plan, managed service organizations (MSOs), physician groups, accountable care organizations (ACOs), and independent practice associations (IPAs).”

George established a relationship with Chetu’s development team at an annual healthcare conference. He decided to partner with the global custom software development company after learning about the extensive industry experience that the company brings to the table.

The Skilled Approach

Chetu’s skilled engineers were tasked with building an MVC5 dynamic web application. Their approach on this project began with creating a database using SQL Server as per the application’s requirements after analyzing the different types of documents the client provided. The application was built on .NET Framework 4.6.1 using Visual Studio. Our developers used Bootstrap, a powerful, feature-rich, front-end development tool that delivers a responsive UI.

During the next development phase, the team began work on the user registration module for patients, providers, different business types, and admin. Afterward, other modules were added that included login, user management, and user permissions, as well as a dashboard that comprised options for sending orders, orders for signature, order review, and order declination. The order declination feature allows for communication between parties to facilitate expedient resolutions.

At its onset, this project had a smaller scope; the application was intended for the use of home healthcare agencies and home medical equipment providers only. As the scope expanded to include utilization by all healthcare organizations, the project timeline was extended. Currently, the project is in the alpha testing phase, soon to be in beta testing. Future plans include HL7 FHIR integrations to maximize the application’s interoperability.

Technology Advances Improved Patient Care Delivery

Chetu’s skilled engineers brought George’s vision to life. Although it is in the final testing phases, the project has been deemed an overall success. The DrOrdrz application enables providers to see all patient care history in one place, providing one central location to store and track documents that require signatures. It also allows for secure messaging between users to help expedite patient care and prompts providers through a billing cycle to facilitate timely invoicing. The application has a clinical dashboard with tools that enable data and cost analytics from the point of care, tracking critical data for organizations responsible for managing the care of patients. It tracks all ancillary care, x-rays, labs, surgeries, hospice, and more to provide total transparency. With DrOrdrz, there is no need to sift through multiple data sources for critical and time-sensitive information, as it offers effective utilization management.

When asked to describe his experience working with Chetu, George had nothing but positive remarks, stating:

“The developers have done a great job. Overall, Chetu’s developers have been very creative and agile; they worked well and gained an understanding of the inner workings of the American healthcare ecosystem, learning the internal financial system to understand the relationships between Health Plan MSOs and IPAs, which is tough because it gets very complicated.”

“Since the commencement of the project, the team has excelled at communicating about its progress, always keeping me up to date and informed so there were no surprises. And when we decided to expand the project, the team easily pivoted to scale their processes in such an efficient way there was no time wasted.”
